HLP includes blood tests, a secure health questionnaire and a physical exam covering body composition, cardiovascular health, grip strength and body measurements. During your one-hour visit, you complete the exam, meet with a longevity physician and receive your personalized written plan for the next 90 days.
Preparation, visit, plan
1. Pay for the Healthy Longevity Plan and provide your email address. 2. You will receive a link to complete your online health questionnaire. 3. After completing the questionnaire, you will receive an order number for a blood draw at any center of your choice. 4. After your blood draw, our coordinator will contact you to arrange your clinic visit.
During the one-hour visit, your longevity physician reviews your results and physical exam, answers your questions and provides your personalized plan. The plan receives clinical review before you receive it. Our analysis tools help organize the information; your physician makes the medical recommendations.
Your plan connects your test results, physical exam, health history and goals. You leave knowing your priorities and what to do over the next 90 days. Ongoing care through the 12-month HLC Roadmap is a separate service.

Specialty tests, imaging, procedures, medications, supplements, repeat tests and ongoing care are outside the entry service. If further care is recommended, you receive its scope and price before deciding. HLP does not commit you to another service. Any repeat tests are arranged and priced separately, with timing agreed with your physician.
The physical exam takes place at the clinic. After your blood draw, your coordinator contacts you to arrange your visit. Visiting South Florida? Share your travel dates with the coordinator. A remote consultation, if arranged separately, does not replace the physical exam.
After payment, complete the online health questionnaire and blood draw; your coordinator then arranges the one-hour clinic visit.
